Community Resource Counselors

UPCAP is seeking applications for five (5) full-time Community Resource Counselors.  The ideal candidates will provide person-centered long-term services and support (LTSS) options counseling and Medicare counseling to clients in the setting of their choice.  Seeking candidates to serve in one of the following five (5) regions of the Upper Peninsula:

  • Eastern~ serving Chippewa, Luce, and Mackinac Counties
  • South Central ~ serving Delta, Menominee, and Schoolcraft Counties
  • North Central ~ serving Alger and Marquette Counties
  • Western ~ serving Dickinson, Gogebic, Iron, and Ontonagon Counties
  • North Western ~ serving Baraga, Houghton, and Keweenaw Counties

Each position will be remote/work from home and involve frequent travel within the designated areas to meet face-to-face with residents requesting assistance. Requirements

  • A Bachelor’s degree in a human services-related field from an accredited four-year college or university is preferred
  • A community health worker, CNA, SW or RN is desired
  • 2+ years’ experience working with people with disabilities and/or older people in the areas of aging, disabilities services, community health, nursing home or hospital discharge planning
  • Familiarity with the aging network, community organizations, and resources for seniors is desirable
  • Some experience with data entry and management is required
